Assistance with Daily Living

Valued Lives provides individualised supports within your home and in your community.

Assistance with Daily Living covers supports such as Assistance with daily personal activities which, for example, includes support with personal hygiene activities, or supervision of personal daily tasks; these supports help you to live as independently as possible at home, and in the community.

Daily living supports are flexible and customised to your own needs, goals and outcomes.

Support Coordination

Having an NDIS plan can be overwhelming. Having a choice is important to make sure that you get the support you need, how and when you need it. However, the reality of coordinating supports effectively can be complex. Support coordination is designed to minimise the complexity of negotiating with service providers, government agencies and mainstream services.

Our Support Coordination service is delivered by experienced and highly skilled Support Coordinators, many who have a family member with an NDIS plan and is based on developing a relationships to assist you to strengthen your ability to coordinate and choose the support you need to participate fully in life and in the community.

Our Support Coordinators can:

  • Assist you to understand the NDIS; what does it mean for you and how can you use the supports in your plan to have a good life. How do you navigate the NDIS system?
  • What is a good life for you? Helping you to say what it is and how you can work towards is.
  • Assist you to identify the support you need to work towards your goals as outlined in your NDIS Plan
  • Provide information and support you to connect with service providers in a way that allows you to make a real choice about how your support is provided
  • Assist you to explore housing options
  • Assist you to understand what “choice and control” really means and what you can do if things aren’t working in the way you would like.
  • Act as a go-between with you, service providers and the NDIS and help you to develop your confidence to do this.
  • Provide information and linking you in with your local community
  • Work with you to develop skills, capacity and resilience
  • Provide coordination to assist you to respond to a crisis situation
  • Assist you to prepare for your annual review

Valued Lives Are Register NDIS Providers and can provide the following services:

  • Specialist Support Coordination (Level 3 – $186.58)
  • Coordination of Supports (Level 2 – $98.06)

Futures Planning (Individual Counselling)

Personal futures planning is an exploration process of a person and what makes them who they are, their dreams and goals and what a good life looks like for them, and what needs to happen for them to work towards their dreams and goals and build on their vision of a good life. It’s often utilised when a person is at the cusp of a major life change or transition. The process and how it is facilitated is tailored to the individual person and their circumstances.

Valued Lives provides this service as per the NDIS Price Guide rate of $156.16

What does ‘TTP’ mean in the rate information above?

The TPP is a conditional loading. It aims to help providers with any costs incurred during transition to the NDIS.

Valued Lives will charge you up to the maximum rate as stated in the NDIS Price Guide & Support Catalogue rates that are current on the date you receive the support including the NDIS Temporary Transformation Payment (where applicable).
