Coordination of Supports
Having an NDIS plan can be overwhelming. Having a choice is important to make sure that you get the support you need, how and when you need it. However, the reality of coordinating supports effectively can be complex. Support coordination is designed to minimise the complexity of negotiating with service providers, government agencies and mainstream services.
Our Support Coordination service is delivered by experienced and highly skilled Support Coordinators and is based on developing ongoing relationships to assist you to strengthen your ability to coordinate and choose the support you need to participate fully in life and in the community.
Support Coordination can assist you to achieve your goals by:
- Identifying the services that will assist you to meet your goals as outlined in your NDIS Plan
- Coordinating service providers and support
- Liaising with service providers and the NDIS on your behalf
- Negotiating how and when you receive support
- Providing information
- Linking you in with your local community
- Working with you to develop skills, capacity and resilience
- Responding to a crisis
There are three tiers of support coordination available to suit your individual requirements:
Support connection – short-term assistance to source providers to meet your needs and participate independently in your NDIS plan.
Coordination of supports – capacity building to connect and coordinate your supports from a range of sources and in a complex service delivery environment. This is available throughout the duration of your plan.
Specialist support coordination – specialist supports for more complex situations.
Peer Support Connection Café
Our new service gives you the opportunity to bring along your NDIS plan and have a chat with one of our experienced Peer Support Coordinators at Valued Lives.
Our Peer Support Coordinators can assist you to hook up to My Gov and the NDIS portal, provide information around services, links to the community and general support to help you to get the most from your plan.
These cafés are aimed at people who have had their plan approved by the NDIS, have some Support Connection allocated in their plan and are looking for some help to get started. A support connection fee will be charged for this service and in most cases this will be covered by your NDIS plan.
